In New Brunswick, appropriately registered Certified Diabetes Educators (CDE) are able to authorize blood glucose test strips for clients with a Social Development health card.

Certified Diabetes Educators, who wish authority to sign paperwork related to blood glucose test strips, as required by Social Development clients with a health card, must first register with the New Brunswick Department of Health, Chronic Disease Prevention and Management Unit by completing and submitting the application form.  Please print clearly on the form. Once an application has been reviewed, you will receive a confirmation by email. Once you have received notification of approval from the Department of Health, then the CDE will be able to authorize blood glucose test strips as long as their certification has not expired. You must renew your registration as above, when you are re-certified.
